Objective: To explore the safety and efficacy of combining electric coagulation and CO 2 cryotherapy in children whose airways are constricted by granulation tissue. Methods: Eight children (aged 2 to 17 months) developed granulation tissue 20 days to 19 months after receiving a stent implant. Granulation tissue primarily appeared at both ends of the stents and, on average, blocked 46% of the children's airways. Argon plasma coagulation (APC) was used first to burn the granulation tissue (15-20 W for 1-2 s); the APC electrode tip was positioned within 5 mm of the tissue. Next, CO 2 cryotherapy (60-70°C for 1 min) was continuously applied until the entire APC-treated region was frozen. The treated lesions were then immediately removed. Criteria were also formulated to assess treatment efficacy post-operation and during follow-up. Results: The eight children received a total of 35 treatments of APC with CO 2 cryotherapy. The treatment was highly effective for six patients, effective for one patient, and partially effective for one patient. Dyspnea among the patients significantly improved, and two of the patients experienced complete re-expansion of atelectasis. All patients had low to moderate post-operative fevers. In addition, four patients experienced active bleeding after APC, which was treated with an endoscopic injection of epinephrine or a topical application of Reptilase. No cryotherapy complications occurred. Conclusion: APC with CO 2 cryotherapy can safely and effectively treat benign lesions and granulation tissue to relieve airway obstructions in children.
