A 35-year-old gentleman who had a 10-year lower back pain with weakness on both legs was admitted to our hospital. A large hemangioblastoma in conus medullaris was detected by means of magnetic resonance imaging scanning. The tumor size was about 4.8 x 2.0 x 2.0 cm and removed totally. The patient recovered well without any complications. The diagnosis, treatment, and prognosis of spinal hemangioblastoma were reviewed. The microsurgical resection of spinal hemangioblastoma en bloc is the best treatment and can result in good prognosis.
